Appointments and Scheduling

When scheduling appointments with our practice:

• Please arrive at least 10 minutes before your scheduled appointment time
• We require at least 24 hours notice for cancellations or rescheduling
• Repeated no-shows or late cancellations may result in a cancellation fee
• We reserve the right to reschedule appointments due to emergencies or unforeseen circumstances

Payment Terms

Payment is expected at the time of service unless prior arrangements have been made:

• We accept cash, major credit cards, and most dental insurance plans
• Insurance claims will be filed on your behalf when applicable
• You are responsible for any amounts not covered by insurance
• Payment plans may be available for extensive treatments
• Outstanding balances may accrue interest after 30 days

Insurance

While we work with most insurance providers, we are not responsible for determining your insurance benefits. Insurance is a contract between you and your insurance company. We will provide estimated coverage amounts, but final coverage is determined by your insurance provider. Any balance not paid by your insurance is your responsibility.

Consent to Treatment

Before any treatment, you will be asked to provide informed consent. This includes understanding the nature of the procedure, potential risks and benefits, alternative treatments, and expected outcomes. You have the right to refuse any recommended treatment.
